Continuity of Care Document-Plus™

A second product in the Hii™ System suite is the Continuity of Care Document-Plus™ (CCD+™). It's a continuity of care document with advanced decision-support capabilities, which helps improve care by fostering information sharing and care coordination between primary care physicians and specialists working with the same patient.

In addition to managing essential patient data, CCD+ adds two important functions missing from all other continuity of care documents. These added functions:

• Identify associations between lab results, signs/symptoms, and medication side effects to help clinicians determine if prescribed drugs are having an adverse effect on a patient
• Display many lab test panels and imaging studies in a unique (patented) manner that is the most clinically useful way to understand a patient's health problems, risks, and trends.
